FUE (Follicular Unit Extraction)
A punch tool — manual, motorized, or robotic — removes individual follicular units one at a time directly from the donor area, leaving small circular wounds instead of a single long incision. The surgeon then creates recipient sites and places each graft by hand.
- No linear scar — leaves scattered dot-like marks that fade well and allow very short haircuts afterward
- Faster recovery than strip surgery, typically back to light activity within a few days
- Can harvest from body or beard hair in some cases if scalp donor supply is limited
- Slower to perform than strip harvesting for very large sessions, since each graft is extracted individually
- Transection risk (damaging the follicle during extraction) depends heavily on technique and surgeon experience
- Per-graft cost is often higher than FUT/strip, since it's more labor-intensive
Best for: Most patients today, especially anyone who wears their hair short or wants to avoid a visible linear scar.
FUT / Strip Method
The surgeon removes a thin strip of scalp — usually from the back of the head — and closes the wound directly with stitches or staples. The strip is then divided under a microscope into individual follicular unit grafts for placement.
- Can move a very large number of grafts in a single session — useful for extensive Norwood 6-7 cases needing 4,000+ grafts
- Often lower per-graft cost, since strip harvesting and dissection is faster than extracting one follicle at a time
- Grafts can retain slightly more surrounding tissue since the strip comes out as one piece, which some surgeons feel improves survival
- Leaves a linear scar across the donor area, which can show with very short haircuts
- Longer, more restricted recovery — stitches or staples, and more discomfort in the first 1–2 weeks
- Not a good option if you plan to keep your hair very short long-term
Best for: Large sessions where maximizing graft count matters more than avoiding a scar, or patients who keep their hair long enough to cover the donor area.
DHI (Direct Hair Implantation)
A variation of FUE where each graft is loaded into a pen-like implanter tool immediately after extraction and placed directly into the scalp, without pre-making recipient sites first — in theory reducing the time a graft spends outside the body.
- Often doesn't require shaving the entire recipient area beforehand, since implantation is more precise
- Can allow tighter graft placement and density in experienced hands, since site-making and placement happen in one motion
- Grafts spend less time outside the body, which some surgeons believe improves survival rates
- Slower than standard FUE for large sessions, since implantation is done meticulously, one graft at a time
- Requires more specialized training and the implanter tools themselves, so it isn't offered everywhere
- Often priced at a premium over standard FUE without conclusively proven better results in every case
Best for: Patients prioritizing precise hairline design or wanting to avoid a full head shave, and comfortable paying more for it.
Sapphire FUE
Standard FUE, but the recipient-site incisions are made with a blade tipped in sapphire crystal instead of steel, producing a smaller, more precise cut.
- Smaller, cleaner recipient incisions can allow denser graft packing in some cases
- Sapphire blades stay sharper longer than steel, so incision quality is more consistent across a long session
- Some surgeons report faster healing of the recipient area due to the smaller wound
- It's a refinement of FUE's recipient-site step, not a different extraction method — the overall procedure, risks, and skill dependence are otherwise the same as standard FUE
- Typically priced higher than standard FUE for a difference that's hard for a patient to see or verify afterward
- The benefit depends heavily on the individual surgeon's technique, not just the blade itself
Best for: Patients who already understand FUE and want their surgeon's preferred incision method, more than a fundamentally different technique.
Robotic FUE
A robotic arm (such as the ARTAS system) uses computer vision to identify and extract follicular units, guided or supervised by the surgeon, aiming for consistent extraction angles and reduced transection rates.
- Consistent, fatigue-free extraction — a robot doesn't tire over a long session the way a human hand can
- Can reduce transection rates in the right cases, particularly for surgeons still building extraction experience
- Useful for very short or unusually angled hair where manual extraction is harder to judge by eye
- Most systems currently handle extraction only — recipient site creation and graft placement are still done by hand
- Not available at every clinic, since the equipment is expensive to buy and maintain
- A skilled human surgeon with an experienced team can match or exceed robotic consistency, so it isn't a substitute for surgeon experience
Best for: Patients at clinics that have invested in the technology and have an experienced team operating it, rather than the robot being a stand-in for surgeon skill.
So which technique is actually right for you?
If you're not sure, the honest answer is that the technique matters less than the surgeon performing it. FUE — in its standard, sapphire, or robotic-assisted form — is the right starting point for most people today, because it avoids a linear scar and suits a wide range of hairstyles. FUT/strip is still a reasonable choice specifically when a very high graft count is needed in one sitting and a scar under longer hair isn't a concern. DHI, Sapphire FUE, and Robotic FUE are all refinements built on top of FUE's core method — worth asking a clinic about, but not something to prioritize over a highly experienced surgeon using the standard version.
